Seminal vesical cyst in patients with polycystic kidney

1. 40-year-old male, father of 3 children, presented with bilateral flank masses of 2 years' duration and episodes of painless haematuria of one week, a month ago. There was no history of urinary tract infections. Clinical evaluation was suggestive of bilateral renal masses with nodular surface. Urine analysis was normal. Blood biochemistry revealed diabetic state and no azotemia. Abdominal ultrasonography showed bilateral enlarged kidneys with multiple cysts. There were no cysts in liver, pancreas and spleen. Seminal vesicles on either side showed a cyst, right being 2.4x 1.4 cms and left being 2.8x 1.7 cms [Figure 1]. There was no family history suggestive of polycystic kidneys.
2. 37-year-old male, married, has no children, presented with history of hypertension since 1995. Evaluation revealed bilateral flank masses confirmed by ultrasonography as polycystic kidneys. Seminal vesicles showed cysts. Urinalysis and serum creatinine were within normal limits. There was no family history of polycystic kidneys.
3. 44-year-old male, father of 3 children, presented with history of bilateral flank masses, hypertension, progressive renal failure since 1993 with a recent history of subdural hematoma operated. Evaluation revealed polycystic kidneys both sides with a single cyst in right seminal vesicle (2.1 x l .1 cms) and small cysts in left seminal vesicle. Serum creatinine was 5.4mg%. There was no family history of polycystic kidneys.
4. 40-year-old male, known patient of diabetes mellitus of 6 years', duration and hypertension and bilateral renal masses detected 1 year ago. Evaluation revealed bilateral polycystic kidney disease and cysts in both the seminal vesicles by ultrasonography and renal insufficiency (S. creatinine 9.5mg%).

Adult polycystic kidney disease is associated with formation of cysts in kidneys, liver, pancreas, spleen, ovary, testis, epididymis, and thyroid. Association of seminal vesicle cysts with polycystic kidneys is reported as a rarity. [1],[2] It is likely that the basement membrane defect allows cyst formation in multiple organs presumably including seminal vesicles.
